Speech Language Assessment Checklist for Adolescents
This checklist was created to assist speech language pathologists in screening and identifying language weaknesses/deficits in adolescents 12-18 years of age who may require assessment and intervention services. It should be given to both caregiver/s and classroom teacher to fill out. Depending on the student’s age and ability, the form may also be given to the student directly, so s/he can identify own areas of difficulty as well. It is important to give this form to several individuals to ensure consistency of deficits, prior to determining whether an assessment is warranted and if so, which assessment areas need to be targeted.
